Program Mission Statement:

Diabetes is a growing problem in the American Indian communities. The disease had reached epidemic proportions and some tribes have the highest rates of Diabetes in the world. However, despite the growth of the disease, resources for caring for those affected by diabetes and preventing this disease are still lagging behind the need.

Our mission is to work collaboratively with the current Indian Health Service Model Diabetes Program and the Leech Lake Fitness Center and Foot Care program to improve the health care and health status of all participating patients with Diabetes on the Leech Lake Reservation.
